Consultation and laboratory evaluation are used to determine qualification for testosterone replacement therapy.
*Level 1A: Includes Testosterone Replacement, Estrogen blocker, along with the required laboratory panels*
Level 1A Plus: Includes Testosterone Replacement, Estrogen blocker and Enclomiphene along with required laboratory panels
Level 1B: Includes Testosterone Replacement, Estrogen blocker. Patients are responsible for obtaining the required laboratory panels at provider’s request
Level 2A: Includes Enclomiphine and the required laboratory panels
Level 2B: Includes Enclomiphine. Patients are responsible for obtaining the required laboratory panels at provider’s request
Level 4: Includes Viagra/Cialis only – No Testosterone Injections
All Memberships include provider assessment and consultations. Patients must qualify in order to obtain treatment
